By logging into the Definitive Healthcare database, I agree that I am an authorized user and that the User ID and Password I am using was assigned to me personally by a Definitive Healthcare rep. I further agree that I am employed or authorized by a firm that has a valid license agreement that is currently in force. I agree that I will not use the Definitive Healthcare data or product, except as authorized by the applicable license agreement and will not share any Definitive Healthcare data with any person or entity other than authorized users from the Company identified in the Definitive Healthcare license agreement.

I further understand and agree that Definitive Healthcare may contact me from time to time to inform me of important information which may include updates and features of the product, training and other educational opportunities, extended use cases and suggestions for product use and new products and functionally that Definitive Healthcare is offering to its customers.
