Remote patient monitoring (RPM) is a method of care delivery that leverages technology to monitor and gather clinical data on patients outside of a traditional clinical care setting.

For example, a patient with COPD could use a handheld spirometer or pulse oximeter or even a wearable device at home to monitor vitals and share with their care team. The care team can then engage with patients via telehealth if vitals worsen to address the issue before needing to go to the hospital.

Patients with chronic conditions can especially benefit from RPM. Apps, wearables, and other devices can help prevent complications and present opportunities for earlier intervention. And by reducing the need for in-person visits, RPM can make care more accessible for patients living in rural areas or those with limited mobility.

Using data from the Atlas All-Payor Claims database, we determined what the most common RPM codes of 2023 were.

What were the most performed remote patient monitoring (RPM) procedures of 2023?

CPT code 99457 takes the first spot on our list, accounting for 36.40% of RPM procedures in 2023. This code signifies billing for the first 20 minutes spent by healthcare professionals on managing a patient’s care based on tracked RPM data. Physicians can spend this time reviewing and analyzing the data, communicating their findings with the patient, educating them on how to use the RPM device, coordinating care with other team members, and more.

In second place, with a share of 30.20% of RPM procedures, is CPT 99454. This code covers the monthly service of monitoring a patient’s vital signs or general health data remotely. The code also encompasses the cost of providing the RPM devices and the transfer of data from the device to the provider’s RPM platform. Unlike some other codes, CPT 99454 can be billed only once per patient, every 30 days, regardless of the number of RPM devices the patient uses.

In third place is CPT code 99458, which was 16.30% of RPM procedures performed in 2023, according to data in the Atlas All-Payor Claims dataset. This code signifies billing for each additional increment of 20 minutes a healthcare professional spends reviewing RPM data after the first. Because of this, CPT code 99458 is an add-on code specifically used with CPT code 99457—a physician can’t bill for 99458 by itself. There’s no limit to the number of times a provider can use this code in a month so long as the additional time is documented.
