Most common primary care diagnoses and procedures in the U.S.

Primary care providers (PCPs) are an integral part of the care continuum. These providers are on the front lines of population health management preventive care, gaining the trust of patients and families to deliver effective, personalized treatments.

PCPs are responsible for chronic care management, specialist referrals, and the evaluation of emerging health issues. Without a PCP, patients are at risk of receiving more serious diagnoses at later stages of illness, which can severely impact care outcomes.

Using data from the Definitive Healthcare Atlas All-Payor Claims product, we’ve compiled lists of the ten most common primary care diagnoses and procedures, ranked by their percentage of all primary care diagnoses and procedures in the U.S. Data is representing claims for the entire year of 2024.

What is the most common diagnosis in primary care?

This table highlights the top 10 most common primary care provider diagnoses in the U.S for the calendar year 2024.

The majority of top diagnoses—hypertension (4.8%), hyperlipidemia (3.6% combined across E785 and E782), and Type 2 diabetes (1.2%)—are chronic, lifestyle-related diseases. Together, they account for over 9% of all primary care diagnoses.

Two of the top five codes, Z0000 (general adult exam, 2.1%) and Z23 (immunization, 1.4%), reflect preventive and maintenance visits rather than illness-driven encounters.

Opioid dependence (F1120, 1.3%) ranks sixth overall, showing that substance use disorders remain a common issue seen in primary care settings.

What procedures are most performed by PCPs?

The top two procedures, CPT 99214 (8.8%) and 99213 (6.7%), together account for over 15% of all PCP procedures, underscoring that the core function of primary care remains patient evaluation, medical decision-making, and care coordination.

99214 represents moderate complexity visits, often for patients with chronic conditions requiring ongoing management, while 99213 covers lower complexity follow-ups.

Codes 3074F, 3078F, and 3079F are all related to blood pressure measurement and control in patients with hypertension, diabetes, CKD, or CAD. Collectively, they account for about 7% of all PCP procedures for the calendar year of 2024.

Venipuncture (CPT 36415) remains among the most frequent procedural activities.
