Largest IDNs in Alabama
The 5.1 million residents of Alabama rely on 128 hospitals and healthcare facilities, many of which are part of a broader integrated delivery network (IDN).
Using the Definitive Healthcare HospitalView product, we created two lists: one of the largest IDNs in Alabama based on their number of affiliated hospitals, and another ranking Alabama IDNs based on their net patient revenue, or the amount of money they receive from patient services.
Geographic reach of Alabama IDNs
As of February 2026, the Alabama health system with the largest hospital footprint is Encompass Health Corporation, headquartered in Birmingham, with 176 hospitals. Encompass Health operates rehabilitation hospitals, with facilities spanning 36 states and Puerto Rico, and ranks among the top 10 IDNs in the U.S. by hospital count.
UAB Medicine and Huntsville Hospital Health System each operate 14 hospitals, making them the second-largest IDNs in Alabama by number of hospitals. Huntsville Hospital Health System, a not-for-profit community-based network, serves patients in North Alabama and southern Tennessee, while UAB Medicine is a non-profit academic health system anchored by its flagship tertiary and acute care facilities in Birmingham and affiliated hospitals across the state.
Other Alabama IDNs with a moderate hospital presence include Baptist Health (Birmingham), Infirmary Health System (Mobile), UAB St. Vincent’s (Birmingham), and Noland Health Services (Hoover), each with five hospitals. Smaller systems, such as Marshall Medical Centers and Helen Keller Hospital Health System, operate two hospitals each, providing care in more localized regions.

Which Alabama IDN has the highest net patient revenue (NPR)?
As of February 2026, Encompass Health Corporation (Birmingham) has the highest net patient revenue (NPR) in Alabama at $4.98B.
UAB Medicine (Birmingham) ranks second with $4.20B in NPR, followed by Huntsville Hospital Health System (Huntsville) at $2.34B. Together, these three systems represent a significant concentration of financial scale within the Alabama IDN landscape.
Mid-sized systems by NPR include Infirmary Health System at $978M, Baptist Health at $966M, USA Health at $964M, and UAB St. Vincent’s at $890M. Several additional regional systems report NPR between $200M–$885M, while smaller networks such as Helen Keller Hospital Health System report $106M.
Notably, the top three systems by NPR—Encompass Health, UAB Medicine, and Huntsville Hospital Health System—also rank among the largest Alabama IDNs by hospital count. However, differences in NPR relative to facility count highlight that hospital volume alone does not determine financial scale.
For commercial teams, NPR should be evaluated alongside additional indicators such as referral patterns, service mix, and network integration to develop a comprehensive view of market opportunity.
Why IDN size matters for healthcare commercial teams
IDN size matters because it determines the impact of a commercial team’s efforts. Larger networks consolidate influence across multiple facilities and providers, requiring multi-level engagement and resource investment, while offering substantial revenue potential. Smaller networks are operationally simpler, with fewer stakeholders and less complex structures, but offer limited scale. Understanding the scale of an IDN allows commercial teams to allocate resources efficiently and target support where it will drive the greatest return.
