What is the Coordinated Care Reward Program?

The Coordinated Care Reward Program is a Centers for Medicare and Medicaid Services (CMS) initiative that sends a $25 payment directly to Medicare beneficiaries who received their Annual Wellness Visit from a healthcare professional participating in the Next Generation ACO Model during the year of 2017 or 2018.

Why is the Coordinated Care Reward Program important?

The Coordinated Care Reward Program incentivized patients to receive regular preventative care, which is known to be conducive to a healthier life with fewer complications and fewer hospitalizations. CMS recognized the importance of the Annual Wellness Visit, and the value it can provide for an individual, and chose to reward those that participate in preventative self-care.
